I confirm that the personal and medical information provided during my initial assessment is accurate and up to date. I am aware that this information is stored on Physio Sport’s system.
Treatment Consent
I consent to physiotherapy treatments, including manual therapy, spinal manipulation, acupuncture, electroacupuncture, cupping, and any other techniques used to address my condition. I understand the potential risks, such as bruising, bleeding, soreness, infection, dizziness or fainting and acknowledge that I can stop treatment at any time.
Media Recording Consent
I consent to being recorded (via video or photograph) during my treatment for educational and promotional purposes. I understand that the recordings may be used on Physio Sport’s social media platforms, website, or other promotional materials.
Payment, Refund Policy and Cancellations
All prepaid sessions are valid for 6 months from the date of purchase. Sessions are non-refundable and must be used within this period.
Cancellations with less than 24h notice will be charged in full.
GDPR Acknowledgment
I understand that my personal data is protected under GDPR, and I have the right to access and update my information at any time. My data will not be shared without my consent, except as required by law.
